Blue Card - Eligibilty

2024-25 Activities Participation Requirements

Any student who plans to participate in activities during the 2024-25 school year must complete the following requirements prior to the first day of practice (for sports).

All students need a BLUE CARD from the activities office in order to participate in athletics.  This needs to be completed before the first day of practice for a sport (Fall, Winter, Spring).  

A new blue card needs to be picked up at the start of each season. 

To receive a blue card, the following items must be completed and on file with the Activities Office.

PHYSICAL

1. A physical completed after May 1, 2024 must be on file in the Activities Office.  Blank copy if needed

PARENT VERIFICATION

2. A parent must complete Student Verification using the Parent Portal to sign handbook and NSAA consent forms. Parent Portal Link

STUDENT VERIFICATION

3. Students must also complete Student Verification to sign handbook forms and NSAA consent forms. Parent Portal Link Students username is their student id number and their password is the same as their computer password.

ACTIVITY FEE

4. Purchase an activity ticket ($70) using the school store, School Store Link, or by bringing in cash or check to the Activities Office.
